The National Health Service Reform (Scotland) Act 2004 required NHS Boards to submit schemes of establishment for Community Health Partnerships to Scottish Ministers for approval by December 2004. Schemes of Establishment lay out the services that will be included, the health and social care services that will be managed by the CHP and the size and scope of the CHP.
