			<content:encoded><![CDATA[<p>The current Community Health Partnerships (CHP’s) in the northern Highland area of NHS Highland – North Highland CHP, Mid Highland CHP and South East Highland CHP – will be combined.  This CHP will be called the Highland Health and Social Care Partnership and will cover the entire population living in the area defined by the Local Authority boundary of Highland Council.  The total population of the Council area is 220,500 and covers a total of 68 GP practices, 58 dental practices, 52 pharmacies and 30 optometry practises.</p>
<p>NHS Highland and Highland Council have approved a commitment to a pathway for integration of health and social care services that would provide both Authorities with the joint responsibility for specifying the outcomes to be achieved for service users and the totality of the resources to be allocated to each of the two service areas.  </p>
<p>The Board and Council has agreed to implement a single lead agency model to deliver the integrated Partnership: for the delivery of Adult Community Care Services the lead agency is NHS Highland; and for the provision of Children’s Services, the lead agency is Highland Council.</p>
<p>The operation of the Health and Social Care Partnership will be internally reviewed after one year of operation.</p>

			<content:encoded><![CDATA[<p>NHS Greater Glasgow and Clyde received ministerial approval to form a single CHP for Glasgow City. Therefore, there are now six CHPs within NHS Greater Glasgow and Clyde.</p>
<p>In 2006, five Community Health and Social Care Partnerships (CHCPs) were created, covering East Glasgow, North Glasgow, South East Glasgow, South West Glasgow and West Glasgow. These CHCPs have now merged into Glasgow City CHP and will be sub divided into three management sectors at local level.</p>
<p>Our website will be updated shortly to reflect the new structure as well as the new contact details for Glasgow City CHP. </p>
